I won't personally tell you until some have answered.
I didn't include 2D dungeons because I don't think they the atmosphere to actually frighten someone.
Honestly, I cannot choose between Shadow Temple or Bottom of the Well.
I would seriously almost scream. :P
Sounds like me, but add in the Earth Temple, and you've got yourself my input.
The "Earth Temple" option is already there.
I say either the Shadow Temple or the Twilight Palace. The Shadow Temple stands out in my mind more, and I can't remember exactly why I thought the Twilight Palace was creepy.
Maybe the hands DW?
I found that The Forest Temple in OoT Was scariest...
The music just sounds like some distant... Thing, is laughing and Taunting you, Combine that with Poes a creepy layout and Wolfos...

What hands? The thing with TP was it moved so fast and had so much stuff in it that I can't remember half of the stuff in it. I am working on beating it a second time... but anyway, that's beside the point. I think part of the reason why I thought the Twilight Palace was creepy was that weird noise that the Twilight people made when you talked to them.
The hands when you pick up the Sols.

But still, nothing stated thus far is as creepy as the Shadow Temple and/or Bottom of the Well from OoT.
And just to bring the creepiness home, think about this: people have lived in that village for a very long time, we know this because lore of the person who once lived on top of the well was somehow passed down, meaning that it wasn't just one person living there.
And to make it creepier, that person had the Lens of Truth, but at one point in time went down and buried it deep beneath the well. Assuming that he wasn't insane or fearless, this means that the torture chambers and stuff were created within Kakiriko's recent history.
Toss up between OoT's Forest Temple, Shadow Temple, and Bottom of the Well, or at least when I first went through them.
I'd say bottom of the well (The dead hands or whatever first appeared) and Forest temple for me.
Shadow wasn't that bad.
I'd have to say it's a tie between the Shadow Temple, Bottom of the Well, Earth Temple, and Palace of Twilight.
Shadow was creepy enough to get my heart pumping.
Bottom of the Well was mad scary with invisible holes, creepy looking fountain,odd wall markings, and rooms with the wood crosses in them that were covered in blood and appeared to be torture devices. Take that and add redeads, bubbles, and Deadhand, and you've got one freakin' creepy place.
Deadhand gave me nightmares...
The Earth Temple wasn't scary as it was paranoid. I was always afraid a poe would come out of nowhere and grab me. ;)
AND THE COFFINS!!! I was soooo paranoid, I insured each one had been thoroughly checked for redeads.
The Floormasters are the worst though! They would always sneak up behind me and grab me! Once, I was eating pretzels while playing and a floormaster grabbed me without warning. I jumped so high the pretzels flew across the room and fell on my bed. I still don't think I got all the salt out of my bed! ;)
The worst part of the Palace of Twilight for me was the Hand. You could always hear it behind you as you panic to find a way out. The weird sound it made still gives me goosebumps. Really!
And then there's Jabu Jabu's Belly... :P

Bottom of the Well was far more creepy than the Shadow Temple, in my opinion. Perhaps because the appearance of Child Link roaming throughout the dungeon had a stronger impact than Adult Link roaming through the Shadow Temple.
It's the well for me. Shadow Temple just had a dark feel to it, but the well was plain down twisted, it felt as if something really gory had just happened there.
Such as a creation of a dead hand.
And yeah, now that I think of it, it was scarier watching young Link go through it than If old link had done it.
The Stone Tower creeped me out a lot when I played MM...but that's not up there, so.
Hmm, I think it would have to be the well. The Twilight wasn't so scary as much as dark and a bit creepy.

I think those were in the Shadow Temple too, perhaps to a lesser extent though. Then there were all those other torture devices / killing machines, like the guillotines and the scythes.
I never found the Shadow temple scary.
I mean you are an adult, you have the sword of evils bane, and all kinds of stuff you can't use as a kid.
As a kid going through Well you have kokiri sword, deku sticks and nuts, slingshot, bottles and a boomerang.
Not that big of an arsenal.
