Yes, he is crying blood, and that is blood on his forewings (or so I have read on the latter).

I have heard that in the original Japanese version of Kirby 64, one of 02's attacks involves shooting blood at you. I believe it was edited in the Americanized version so that it was just sparkles of energy or something. Or it could've been changed to green like in Ocarina of Time when Ganondorf coughed up blood, because green cloud attacks are one of the attacks he does when you attack his cactus-like tail.
